The lyrics refer to the Nazi 'physician' Josef Mengele, who conducted hideous human experiments at Auschwitz during World War II. Slayer detail Mengele's physical and psychological atrocities in graphic detail including experimental surgeries performed without anesthesia, transfusion of blood between twins, isolation endurance, gassing, injections with lethal germs, sex change operations, the removal of organs and limbs. A deplorable moment in human history put to music, it is understandable that the track received widespread condemnation from Holocaust survivors and accusations of Nazi sympathizing and racism has dogged Slayer throughout their career. Even the record label, through fear of reprisal, distanced themselves on release. Grisly subject matter written to shock and appall this was idle fodder for an extreme metal band, Slayer just happened to make the biggest impact.
