Speaking on The Wrestling Inc. Daily podcast, IMPACT Wrestling's Brian Myers (a noted ECW fan whose new podcast, Extreme Conversations, covers the cult promotion) revealed that while Sandman still uses cans of beer as part of his famous ring entrance, he isn't drinking anymore (h/t Wrestling Inc.):-

“I don’t know if this is public knowledge but he’s sober right now. So if you watch him on an indie show or whatever, he’s just kind of making a mess of beers but he’s not consuming.”

Said beer cans were a huge part of Sandman's aesthetic. Walking out to Metallica's Enter Sandman, the five-time ECW World Champion wouldn't just take swigs from them, but smash them across his forehead, often drawing blood. In 2021, however, it appears the beer is all for show.

Now 58 years old, The Sandman works only a sporadic wrestling schedule. His last match went down on 3 October 2021 for indie promotion World Class Revolution, with his most notable opponents over the past few years being Shannon Moore, Killer Kross, and Gillberg.

Sandman was with ECW on and off between 1992 and 2001, becoming the company's biggest cult hero in that time. He later wrestled for XPW, WWE, and TNA, before returning to the indie circuit.
