4. Catch-22 (1970)

Based off of the critically acclaimed novel, this film was about war and what it does to people, but in a comedic light. It wasn€™t a very popular movie, but was responsible for a very horrific death of a very important crew member. The second unit director, John Jordan, was sucked out of a plane during filming and plunged several thousand feet to his death. Jordan was absolutely responsible for his own death as he was not wearing the safety harness that was required in order to ensure everyone would be unharmed during filming. Not only was he not wearing it, but he refused to wear it. It is unclear as to why he refused, but demonstrates the importance of following safety measures. It is also a shame to know that such a terrible thing could happen on what was an otherwise funny set.
