20. Brenda - Bled Out(?) (SAW IV)

Brenda was a prostitute going into Saw IV and found herself placed in the film's infamous and terrifying scalp trap. Locked in a device that pulls her hair and slowly starts to rip off her scalp, Rigg ignores Jigsaw's warnings to leave her alone and is eventually able to set her free. Instead of being grateful for her rescue however, Brenda goes on the offensive and tries to attack Rigg.

As it turns out, Brenda had been ordered by Jigsaw to attack Rigg should he ignore his orders of leaving her to her fate. Rigg manages to eventually overcome the prostitute, and throws her through some glass. In what has to be one of the most WTF moments in the entire history of film, Brenda's fall through the glass ends up being a screen transition into an entirely different location.

The lead up is pretty bloody and the game on the whole is a real tough watch given how we could all imagine the sheer amount of pain ribbing someone's scalp off would induce, however that transition on its own is just too crazy to not include in the of this list.
