3. Face Ripped In Half - Phantasmagoria

1995's Phantasmagoria is by no means a good game, but it does deliver one of the most nauseating pieces of video game violence I've ever seen, made all the more disturbing because most of the game's content is, of course, live action. Coming from the era of point-and-click adventure games that attempted to immerse the player more through a series of poorly acted, usually cheap-looking film clips, Phantasmagoria was no different except for in one area, and that's the unexpectedly brilliant gore effects, which still hold up pretty well today, almost two decades later. As the game's protagonist, Adrienne, enters a room, a demon shows up, grabs her by the eyes and then proceeds to effortlessly rip her face in half, revealing the red, gooey cavities inside. It's a brief shot, but one that's extremely well-executed and certainly wouldn't seem too out of place in a horror film nowadays.
