This achievement has actually become more controversial over time due to how the world has changed since the game's release.
Though the achievement requiring players to urinate on an NPC until they vomit is gross enough, the 'cheev's title is a reference to a video of R. Kelly having sex with and urinating on an allegedly underage girl, which was uncovered shortly before Postal 2's 2003 release.
While Kelly was acquitted in 2008, the release of the last year's docu-series Surviving R. Kelly renewed interest in making Kelly accountable for his transgressions, resulting in a new trial for sexual abuse allegations being set for May 2020.
In the early 2000s it was decidedly less frowned-upon to crack jokes about sexual abuse, believe it or not, but almost two decades on, especially in light of Kelly's very probable impending conviction, it's tough not to wince at the achievement.
