Completing Modern Warfare on Veteran difficulty is already a lot to ask of a player. With enemies having pinpoint accuracy and spamming hundreds of grenades if you dare seek out cover, getting through the main campaign on the game's hardest mode was one of the biggest challenges that the previous gaming generation had to offer.

Because just getting to the credits didn't mean that the torment was over, as the sequel saved its most punishing task for last. To unlock the Mile High Club achievement, all players have to do is get to the end of the titular level; by shooting through cramped rooms with no cover, all the while a time limit of one-and-a-half minutes continues to tick down.

It's ridiculously demanding, and despite the level being shorter in length than most pop songs, it will still take you hours and hours to find the right pattern to eventually brute force your way through.
