In Mother Base, near one of the landing zones you'll find a scaffold reading "Caution: under construction", with a picture of boxes falling on someone's head. Go over there and stand still for a few seconds, and you'll befall the same fate, a box falling on your head and knocking you to the floor like a sack of potatoes, complete with a hilarious "boing!" sound effect.
