Genuinely, during any Captain/Warchief/Overlord encounter you might come across an enemy who - if you get knocked down and miss the counter/stay alive prompt - will literally grab your existing blade and snap it in half.
Though you'd then assume it's gone for good, seek out the new "Vendetta" mission that's popped up relating to said orc, and complete it.

Upon defeating your newfound nemesis (do not recruit them) they'll drop the stolen gear again - thankfully with additional levels that match whatever upgrade they received for killing you in the first place.
