They never saw it coming.
Doctor Who is steeped in death and it is very rare for an episode not to have someone shuffle off this mortal coil. It's so notable in fact that we often remember when, such as that two part oddity aboard the TARDIS Inside the Spaceship/The Edge of Destruction (where the Hartnell crew get techy with each other and Susan waves some scissors about) a tale where no-one actually perishes. Some stories have had their cake and eaten it, having people die horrible by being turned into gas masked zombies and then had some magic pixie dust blown at them so that 'Everybody lives!' However, Doctor Who usually ends with a sizeable body count as people and aliens are exterminated by laser, converted by Cybermen, sonically blasted by Ice Warriors or simply shot by a human adversary. After a while many of these demises can get a little repetitive, boring even.
