15. An Unfortunately Placed PG Graphic
We get that overlay graphics for TV programs are formatted in advance, but somebody should have noticed before SmackDown that WWE's tribute to the recently-deceased King Kong Bundy would be ruined. Right as the "In Memory Of" spot showed up, so did a "TV PG" stamp that plastered over the WrestleMania II headliner's face.
