How Did He Die?

Speaking of the Proudstar clan, Warpath also had a temporary brush with death. In shutting down the evil experiments of a mad doctor named Martynec (one of whom's experiments was Jimmy's mother), Warpath was injected with some nasty sh*t that caused him to go into cardiac arrest and die.

How Did He Come Back?

Siryn performed CPR on him, but the real kicker was Valkyrie leading a team of his X-Force squadmates into Hel to get his soul back. The two events combined helped bring him back.
