I can only speak to what I've found, but Lady's signature Kalina Ann missile launcher can be found when playing as Dante in Mission 11, Reason.

Tucked away inside a random world globe, the game pops up a series of question marks onscreen to let you know something is worth investigating nearby. Through sheer luck I happened to swipe the globe in half, auto-picking up the Kalina Ann inside.

I won't spoil the story-integral evolution of this weapon, but you can get the drop on enemies with one of the most powerful firearms in the game if you keep your eyes peeled.
