Given that Thor's new weapon in Infinity War was made specifically to be a Thanos killer, it didn't do its job because of where the God Of Thunder chose to hit him with it. Putting aside all concerns about it failing to do its one job, you really have to wonder how Thanos managed to survive such a blow. Look at it again, it basically caved his chest in and yet in his very next scene, he's pretty much uninjured.

The answer comes if you look at the moment leading right up to the snap closely. Just before he teleports away, the smallest hint of green sparks around the wound in his chest. In other words, Thanos used the Time Stone - almost unconsciously - to reverse the injury. He essentially pushes the thunder-axe out of his chest and closes the wound. That's why aiming for his head was the only way to kill him - he couldn't guide the Stone to heal himself.
