4. Everybody In David's Car - The Mist

This one really hurts. Frank Darabont's The Mist is a terrific sci-fi horror film in which Earth is invaded by inter-dimensional, Lovecraftian monsters.

It's best remembered for its jaw-dropper of an ending, in which protagonist David (Thomas Jane) enters into a suicide pact with the other remaining survivors after believing they've got no hope of survival.

David then shoots the other four survivors dead, including his unknowing young son, before turning the gun on himself and realising he's out of bullets.

But as David steps out into the mist to be devoured by the monsters, a tank rolls through the area instead, revealing that the U.S. Army has regained control and is in the process of exterminating the monsters.

Basically, had David and company just waited like 90 seconds longer, they all would've survived. Instead, David had to go on living with that knowledge, and probably ended up in a psych ward shortly after the events of the movie.
