From the humble beginnings of Saw to the blood-spattered sadism of Hostel, torture films are the home of some of the most deprived and intense sequences ever put to film. But out of this collection of twisted monstrosities, one stands out as being the most sickening of all: Grotesque.

Banned in the UK for its focus on torture and humiliation, this film sees a young couple (Kotoha Hiroyama and Hiroaki Kawatsure) abducted by a mysterious figure known only as The Doctor (Shigeo Osako) and systematically tortured for their captor's own twisted pleasure. During their ordeal, the pair endure amputations, eye gouging, sexual assault, and even castration. The sadistic doctor ensuring to treat their wounds to keep his victims alive.

However, it’s the doctor’s last test which proves to be the most grotesque of all.

Cutting the man's stomach open and attaching his intestines to a hook, the doctor tasks him with collecting scissors so that he can cut himself and his girlfriend free. Every step is a struggle as he inches closer to the scissors. With a lot of blood and vomit involved in this arduous sequence, enduring this scene is its own kind of torture.
