Everyone who took on the mantle of Jigsaw had their own spin on things. Jigsaw gave people the chance to fight for their lives, Amanda built inescapable traps, and Hoffman tended towards traps where one safe person would decide the face of others.

This trap was one of those. The victim is a publicist who spread lies, and her client has to pull a key out of her stomach, attached to a fishhook, to free her from a device which would impale her throat. Any noise above a whisper would just speed up the impalement.

Screaming is unconscious. It's not just about speeding through this- he has to speed through it and she has to fight every natural urge, every instinct, and stay quiet.

She fails.

This trap's disturbing because it's not enough that she goes through agony. It's that she loses the relief, however small, screaming would have afforded her.
