2. Malignant - Gabriel
It is understandable that the writers of today are keen on throwing plot twists into their movies - a great plot twist can literally save an entire film single-handedly - and there's probably no better example of this from recent years than Malignant, which centres around Madison (Annabelle Wallis), who is having visions of a series of grisly murders which are happening for real nearby.
For the first two acts, Malignant is a visually stunning yet unevenly acted and badly scripted slasher homage. And then, at the start of its third act, it deploys its secret weapon: one of the most outstanding twists in recent memory.
Madison has a parasitic twin inside her named Gabriel, who has now become sentient and is a psychopathic killer that's been carrying out the murders by hijacking Madison and operating her body backwards. After this reveal, the film abruptly transforms into a gloriously unhinged slice of awesomeness and delivers a wonderfully bonkers final act in which the fully revealed Gabriel goes on a terrifying, gore-soaked rampage.
Horrifying, bonkers, blind-siding, creative, enthralling, all of the above - this plot twist is practically perfect.
