Hereditary centres around Annie Graham; a family woman dealing with the recent death of her secretive and unstable mother, Ellen.

After her teenage daughter, Charlie, is decapitated in a freak accident, Annie encounters a support-group member called Joan who tries to help her deal with her grief. At first, Annie assumes meeting Joan was a chance encounter. However, she eventually learns that Joan was friends with Ellen. Not only that, the pair are demon worshippers who are attempting to bring their master, Paimon, to Earth by using Annie's son, Peter, as a vessel.

Realising that Paimon had been manipulating the family through Charlie for years, Annie burns her daughter's diary, assuming this will destroy the demon. As soon as she incinerates Charlie's papers, Paimon has Annie's husband burst into flames, leaving her so vulnerable, the demonic entity possesses Annie with ease.

Using Annie's body, Paimon chases Peter around the house, leaving him injured enough for the demon to enter his body. He then makes his way to the treehouse where demon worshippers are praying to the severed head of Charlie. Joan appears and informs him that his followers are ready to carry out his wishes.
