There's a rich narrative vein in the MCU that sees Tony Stark dealing with the sins of his past. He was a playboy more intent on selling weapons to service his lavish lifestyle than with the collateral damage his choices leaves behind.

Selling bombs to people is one thing, though, because you can easily switch off the thoughts of what they do to people under the guise that you're doing good for your country. Less forgivable is straight up wrecking a guy because he's a bit weird-looking, has a disability and possibly has poor personal hygiene and turning him into a sexy supervillain with a grudge.

In his first meeting with Aldrich Killian, all Tony had to do was be civil, but instead he was a dick and essentially taught Killian that being a dick would lead to success. In business terms, shunning the possibilities of Extremis was idiotic, but to do it in such a way that Killian became a terrorist is on another level.
