During Bond's early escapade in Beirut, where he retrieves a golden bullet from the navel of a belly dancer, he finds himself attacked by several suited-up assailants, leading to a brutal fight scene.

However, when one of the goons is thrown into the belly dancer's dresser table, he knocks the mirror out of position, pointing it towards the camera at such an angle as to quite blatantly reveal the camera crew.

It's an especially epic gaffe because the crew are visible for so long, and in such a huge blockbuster production, that's as embarrassing as it is hilarious.
