While this list is mostly sticking to big-budget superhero movies, the 1989 Dolph Lundgren version of The Punisher makes the cut for committing a cardinal sin beyond compare: refusing to include the character's iconic skull T-shirt.

It's especially galling as Lundgren honestly wasn't a terrible choice to play Frank Castle and the rest of his attire is actually pretty on-point. But the skull is an enduring emblem of the character that even casual comic book fans can appreciate, so its exclusion here is absolutely baffling.

Furthermore, this wasn't some absent-minded oversight but an active decision made during script re-writes. Considering The Punisher's costume is surely one of the easiest to get right in all of superhero lore, it's hilarious how effortlessly they shot themselves in the foot here.
