19. Red Room (1999)
The Plot: Four contestants appear on a reality TV show where they are all locked inside a room and tasked with torturing one another to the death, while the sole survivor walks away with a huge cash prize. Why It's Disturbing: Unsurprisingly, it doesn't take long for the torture to venture from relatively benign to utterly disgusting, as soon enough there's rape, genital biting, urinating in people's mouths, lightbulbs being shattered inside vaginas, and the bizarre punishment of sticking a hair dryer in someone's mouth. This is a fine example of a movie that knows its limitations and makes the best of it: it's only 68 minutes long, but is bracingly intense for the majority of that run-time, mining enormous amounts of tension as the audience tries to figure out what's going to happen next. It's very rough around the edges owing to its low budget, but it's legitimately terrifying and, unlike some entries on this list, actually pretty entertaining.
