The shocking reveal that Malignant's killer is Madison's parasitic twin will no doubt cement its place as a future cult classic. This jaw-dropping revelation may come completely out of left field for neophytes, but savvy genre fans familiar with tradition will instantly recognize the similarities between Malignant and the cult favourite, Basket Case.
Like Malignant, Basket Case concerns the fallout from attempting to separate conjoined twins, and depicts the bloody rampage that ensues. In Basket Case, the protagonist, Duane Bradley, enlists the help of his now-separated twin, Belial, to hunt down the surgeons responsible for the procedure that removed Belial.
The twins of Basket Case are psychically linked just like Malignant's brother and sister duo, and revenge against the surgeons' responsible for the botched surgery that attempted to separate Gabriel from Madison is what motivates the killer.
