To make the whole set-up even weirder, after Bea Arthur heads off to serve other customers, Krelman takes a healthy swig of his drink...by pouring it into a hole in the top of his head.
Sure, it's a bit more dignified than playing a cross-dressing cooking show kook or a spasmodic robot, but not by a whole lot.
As for the character's physiology? You'll need to fill the gaps in on your own, because it's unlikely you're ever going to get canonical answers from anyone.
