Throughout the DLC players are shown the handiwork of a particularly psychopathic surgeon, who's chopping up the all-male inmates of the Mount Massive asylum and trying to transform them into a wife so he can start a twisted family.

Players constantly come across his rejected attempts, all of who have bits missing, bits added, or bits completely switched around. There's some outrageous body horror on display, but it all becomes more than an abstract threat when you're eventually captured by the good doctor himself.

Knowing full well what kind of weird stuff the guy is into, you can probably guess what's about to happen when you wake up stripped naked with your junk pointed directly towards a table saw.

Thankfully you manage to escape, but having to watch as someone less lucky than you gets an unwanted castration and then waking up to face the saw yourself is more than enough to give players nightmares for weeks.
