The Mistake

Bran has been going on some trips to the past with the Three-Eyed Raven, but the Stark boy is getting too much of a taste for them - as the Raven warns him, if you stay too long you'll drown. This leads to a frustrated Bran, tired of sitting in a tree and waiting to go into visions whenever the Three-Eyed Raven dictates, deciding to go off on one by himself, despite the rather obvious and stark (pun only slightly intended) dangers of this. This vision leads him to the army of the dead, where the Night King is able to touch and mark him, breaking the magic protecting the Three-Eyed Raven's cave and giving away its location.

The Death

With Bran essentially serving as a homing beacon for the White Walkers, it's not long before the Night King and his icy cohorts are upon the weirwood tree. Bran and Meera are able to escape, but there's a trail of bodies behind them, with Leaf, Summer, and most devastatingly Hodor - with a time-loop shock thrown in for good measure - all dying to help him get away. The Night King, meanwhile, is able to confront and quite easily kill the Three-Eyed Raven, with Bran now taking on the mantle.
