2. The Good Wife - Kristin Chenoweth

While The Brady Bunch managed to escape the danger of faulty, unsafe equipment, former West Wing star Kristin Chenoweth wasn't so lucky on the set of the Good Wife. She was cast in a recurring role as a political reporter in the fourth season, but in only her first episode, she was left unconscious when a light fixture fell on set during filming and hit her in the face. She sustained a skull fracture, broken nose, spinal and rib injuries and cracked teeth, and said later that she was unable to even form a sentence in the wake of the injury.

Had the blow been more directly on her head, it's likely that her injuries would have been even more extensive, to the point of fatality. After her recovery, she chose not to return to the role, aside from a short appearance in another episode.
