1. Season 6 Episode 1: Fallout
Rarely does an opportunity like this present itself in TV; the opportunity for the showrunners to reshape the landscape and status quo of their show, by having a scenario where any of the shows characters could be killed, except for the main one. In the season 5 finale, Oliver was safely on a boat, whilst everyone else was stuck on the exploding island, leaving any answers of who survived open-ended. The possibility for a positive change for the show was looming.
Unfortunately, the next episode does the absolute minimal amount with this golden opportunity. Every single character lives, except Oliver's baby mama (the only character who was all but confirmed to be dead). This cowardly decision left the show in its bloated, overwritten state, and completely devalued Season 5's brilliant ending. Season 6 could've been one of the most daring seasons of Tv ever put to screen, but instead, it stood as proof that Arrow's creative team likes to remain firmly where it feels easiest and safest.
'Fallout' is the most disappointing episode of the show, because it let its biggest moment to impress slip through its fingers. It epitomises why its writing is so frustrating; one step forward often means two steps back. It was an upsetting let-down, and easily takes this top spot.
