The nickname origin of "Sweet" was revealed in the latest season of It's Always Sunny. Dee was once a charming, young actress about to move to Hollywood, until she falls and hits her head at a skating rink due to Charlie loosening the straps on her skates. She is transformed into her foul mouth alter-ego that audiences will recognise as the current day version of Sweet Dee.
You don't survive 15 seasons with the Gang unless you're a narcissistic sociopath, and Dee is no exception.
