9. Holodeck Malfunctions
Starfleet's Holodecks, introduced in the 24th century, have caused so many accidents and disasters that one has to wonder why they would keep them aboard nearly every ship in the fleet.
Just to name a few malfunctions, there are the holograms who were inadvertently made self-aware such as The Doctor from Voyager and the Moriarty hologram from the Next Generation episode, Elementary, Dear Data, as well as the hologram Badgey from the Lower Decks episode, Terminal Provocations who glitched and became evil and murderous. In the Voyager episode, Bride Of Chaotica!, some photonic lifeforms mistook the ships holodeck simulation for the crew, eventually getting in a war with the hologram characters they encountered.
Let's also not forget the flimsy safety protocols that seem to break all the time, rendering holographic weapons just as deadly as real ones. With so many horrifying possibilities, Starfleet should really look into redesigning this technology to be safer.
