This one is a mind-bender people, so bear with me. After being activated during a ship-wide emergency, the Doctor cannot distinguish fantasy from reality, as nothing is as it seems. He can leave the sickbay because holo-emitters have been placed in strategic parts of the ship, such as the bridge and engineering, yet he and us did not know that was even being worked on.

This should be our first hint that something isn't right. Each time the Doctor thinks he knows what is real, the rug is pulled out from under him and he is once again left to determine if what he is experiencing is real.

Enter everybody's favourite recurring lieutenant, Barclay! He tells the Doctor he is Lewis Zimmerman, the creator of the Doctor's program who has become stuck in a simulation due to a radiation surge and the only way to get him out is to end the program by destroying the ship.

Then along comes Chakotay, who tells the Doctor that he doesn't need to destroy the ship to get himself back to reality, the Voyager crew are working to save him as his program has been affected by (you guessed it) a radiation surge. With Barclay on one side and Chakotay on the other each trying to persuade the Doctor that they are telling the truth, we are forced to wonder along with the Doctor who to trust. As the stakes get higher, so do our anxiety levels.
