1. The Death Of Trench

Despite the previous entry on the list, Trench's actual death left a lot to be desired.

After Anakin stabs him, he stumbles around a little bit before his arms finally fall, as he gives out one last signature click. All the while, the score plays funny music, as if Trench's death were the punchline to some joke.

Trench has been a major Clone Wars villain since season 2. The Separatist Admiral was not a comedic character, and at times was pretty intimidating. He deserved a more serious death than he got, and letting him die right after Anakin stabs him would have been a more dignified end.
