Out on a run with Michonne (Danai Gurira), the pair were kidnapped by The Governor and used as leverage against Rick. When Rick tried to reason with with him, The Governor snapped and decapitated Hershel, sparking a major battle between the two factions.
After the battle, Michonne found Hershel's reanimated head, and put him down with her sword.
