This is just horrific - one of a few entries on this list that serve as a reminder of how dangerous, and worrying, the match is.

One would hope that, presumably, somebody expected Kofi Kingston to either bounce back up from or slide down the rails of the ladder following Kane's powerbomb. This did not happen; instead, with a jarring suddenness, the rails connected with the mat instantly - and since Kofi followed almost as quickly, it was a minor miracle that his head was not caught under the rungs. This was disturbing in a more low-key manner than the more highlight reel-worthy MITB moments, in the sense that it felt like a near-drastic miscalculation more so than a planned sequence.

Kane is a Big Red Euphemism. He's known for his longevity, not a "legacy", and for his record as the "safest" worker in company history. If the match can conspire to see him come close to injuring an opponent, there must be something inherently dangerous about it.
