Tom Phillips caught the same nonsense afflicting Michael Cole's memory at Clash when he proudly announced that Randy Orton was embarking on a "record-tying 14th World Championship" effort against McIntyre. How is that 'record-tying"?

They literally noted that John Cena is a 16-time champ on the same show.

In fairness, Tom corrected himself a few minutes later by saying that Orton could tie Triple for second most in history. Someone may have roared that over the headset to him when Saxton and Joe were running through their own lines.
