Broken launch-day servers, death threats and paying for extra story content - what went wrong?

It's safe to say, as we head into the home stretch of 2014 the journey here hasn't been a smooth one. In fact, looking back the majority of the year appears more like a giant black void with the occasional spike of life through the likes of Watch Dogs or Titanfall whipping people up into a stupor, only to fade away again. The reason for the lull was apparently due to developers not being totally sure the consumer base actually wanted new consoles - only to be caught shorter than Tyrion Lannister when last year's sales figures came rolling in. They've suitably picked themselves back up even though it's taken most of the year, and now as the lead-in to Christmas kicks into overdrive, we're getting a steady stream of must-have titles. Showing that the industry itself is slowly shovelling coal back into the triple-A furnaces and giving us our yearly instalments of both Call of Duty and Assassin's Creed is all well and good, but it remains that the general sentiment shared amongst gamers across the year was that of dusty consoles and wasted potential. It doesn't help that one of our most celebrated developers who so many were relying on to deliver another landmark title in gaming history essentially dropped the ball when it came to customer satisfaction either...
