1. Charlize Theron as Aileen Wuornos in 'Monster'

The before and after pictures of Theron playing, real-life serial killer, Aileen Wuornos must be some of the most famous out there and not because it turned her into an entirely different person but because it turned her from a woman who struts around in perfume adverts into a ravaged version of herself. But the visual change was only part of the performance with the real Wuornos obviously suffering from mental illness, in particular personality disorders, Theron immersed herself in the transformation and her frightening and damaged depiction of Wuornos won her both an academy award and a Golden Globe.

Theron had played 'the babe' in a few films up until 'Monster' but she wasn't type-cast as such it was just that her performance as Wuornos elicited such genuine shock and admiration from the media and the public. If there's anyone who said pre-2003 they saw something like this coming from Theron, aside from the lady herself, I'd be very very surprised.
