The Plot: Two men wake up, chained in a toilet. In between them: a dead body. Say what you want about the Saw franchise, but the central premise of the original remains, well, original. Potential victims of The Jigsaw Killer, the two men become embroiled in his game of complete-a-task-or-die, toiling over the morality of their situation until the films absurd, shocking finale.
The Twist: The body isn't dead. Its Jigsaw himself.
Why Its Great: Absurd though it is, Jigsaws reveal is the last, most shocking trick of a film chock-full of them. To see him rise slowly from the floor after being an almost afterthought in the films milieu is to see one of the Horror genres great moments. This is perhaps the most implausible twist on the list, and the one that loses impact most. But its still a great moment, even more so when you consider the rest of the Saw franchise, which looked to bastardise the originals impact at every opportunity.
