The Basket Case trilogy follows Duane and his former conjoined twin, Belial. Because Belial is a bodiless torso, Duane is worried they'll never be accepted by society.

Luckily, the pair are welcomed into Granny Ruth's home, where she cares for deformed people who have been shunned by society. Even though these outcasts have moon-shaped heads and horse faces, we are reminded that Granny Ruth's son, Little Hal, has the most extreme deformity.

Instead of showing Hal straight away, he's subtly teased for the next hour. In an early scene, the characters admire a painting of Hal as a baby, which depicts him with six arms. Although we see Hal several more times, his body is always obscured.

When a police officer bursts into Ruth's home to arrest her, the group reveal Hal to him, hoping his deformed appearance will freak him out.

But instead of baring witness to the scariest monster imaginable, all we see is a big blob wiggling his fingers. Also, it's obvious his extra arms are just stage-hands (literally).

Worse still, Hal doesn't do anything! He doesn't attack the cop. He just stands there!

It's ironic how the monster who was built up the most happened to have the most uncreative design.
