At the start of the film, we are led to believe that the family is a normal one, like the one Esther encounters in the original. Later on, however, we find out that older brother Gunnar killed the real Esther; a crime that was covered up by matriarch Tricia. This means that the entire time, the mother and son duo were aware of the killer's deception, and for the sanity of grieving father Allen and protecting Esther's own hide, both parties make agree to keep things a secret.

This kind of twist is one that helps the prequel differentiate itself from the original Orphan. By making the main family villainous, it gives Esther a fresh obstacle to overcome despite knowing she'll live to make it to the first film.
