Graham Chapman's alcoholism unfortunately became both a regular fixture and regular problem on the set of Monty Python on the Holy Grail.

Chapman's attempts to sober up during production resulted in him suffering from severe tremors and forgetfulness as he entered withdrawal, which inevitably resulted in him quite literally needing to return to the drink in order to actually perform his scenes properly.

Director Terry Gilliam claims Chapman actually got too drunk at one point, though, and the film's first assistant director had to eventually double for him during the Bridge of Death sequence, as Chapman was physically unable to safely do it himself.
