5. Noah's Ark
This 1928 disaster movie based on the biblical story of Noah and the Great Flood came during the transition period from silent films to the era where sound was available. With a plot line that flipped between the biblical event and the start of World War 1, where the actors and actresses played dual roles, it was well over two hours long and was considered a financial success at the time.
Unfortunately, it was also a watershed moment for the industry as a whole as a number of incidents during filming resulted in more stringent safety measures being adopted in Hollywood and further afield. While the time when it was filmed leads to difficulty in finding corroborating sources, it is generally accepted that when filming the "flood" scene, the amount of water that was released was extreme and resulted in a number of fatalities as well as at least one extra having a limb amputated and others suffering breaks and injuries.
While it is obviously tragic that anyone would lose their life for the sake of a movie being shot, the controls that were implemented following this did indeed lead to incidents like this becoming markedly less frequent in the decades since.
