1. Visible Crew In Kurt Russell's Sunglasses - Stargate

Despite mixed reviews, Stargate was a solid box office success and helped kickstart the blockbuster career of director Roland Emmerich.

However, it touts a glaring flaw in the early scenes featuring Colonel O'Neil (Kurt Russell) and his men on the desert planet Abydos.

In close-ups and mid shots of Russell's face, his sunglasses reflect the film's crew right back at the camera, revealing not only a number of noticeable human bodies but also a giant white diffuser sheet.

This became infinitely more noticeable on the Blu-ray release of the film, noticeable enough that one hopes they might actually give it the James Cameron treatment for the 4K UHD and digitally erase the hilariously blatant set reflections.

They don't break the movie, but once you notice them and then hit the pause button and fixate on them, they'll be on your mind every single time you revisit the film.
