Iron Man is the hero of the MCU, declaring himself a hero in the first film, proving it in the second film, making the sacrifice play in Avengers, and finally finding out who he truly is, in or out of the suit. He declares himself Iron Man as he throws all of his suits, declaring he doesn't need them. He's Iron Man without them.

Then Age of Ultron comes around. It begins his slide into anxiety and insanity that culminates after losing Peter Parker on Titan. Here, Iron Man's PTSD and anxiety have escalated. He's building suits obsessively. He wants to, not only create a brand new AI out of an alien weapon that took over a bunch of people's brains a couple years ago, but put his own robots circling the world, forever.

He goes against his friends wishes, his creation evolves into a monster that almost destroys the entire world, and even with this threat, Iron Man still presses on, deciding to create an even smarter, even more powerful robot. This slow slide into insanity would continue into Civil War with him and Pepper separating, until he finally broke at the beginning of Endgame, and hid from superhero life.

Every bad thing that happened in Ultron was Tony Stark's fault, 100% and it was the beginning of his downward spiral.
