2. A Living Woman Was Included In The In Memoriam

Embarrassingly, a living woman was accidentally included in the In Memoriam sequence. Yes, Oscar-nominated costume designer Janet Patterson did pass away late last year, but that picture is not of her, but rather Australian film producer Jan Chapman.

To make matters worse, Patterson was Chapman's good friend, and was said to be "devastated" by the mistake. Though this was certainly the most offensive and careless mistake of the night, nothing tops how the show concluded when it comes to the sheer WTF factor
