Immortan Joe (Hugh Keays-Byrne) is in pursuit of Furiosa and the Five Wives, specifically The Splendid Angharad (Rosie Huntington-Whiteley), who is heavily pregnant with his child. She places herself in between Joe and Furiosa to stop Joe and his men from firing on them, but she eventually slips underneath the war rig and is run over. Joe's men recover her body, and as she slowly dies, her unborn child is removed from her womb by C-section, leading to this grotesque sequence in which a makeshift doctor plays with the umbilical cord and tells Joe that the baby didn't survive, but that it was "perfect" had it lived. It's not especially gratuitous (we only see the baby for fractions of a second as it's discarded by the doctor), but considering that we could easily just assume that the baby died inside the womb, George Miller very consciously took it one step further.
