Critics were blown away by Captain America: Winter Soldier as it seemed to up the game for the MCU. After six years, it's still seen as one of the best entries in the franchise. However, it's reputation could have been different if they used the original script.

Halfway through the movie, Black Widow and Cap encounter a computer that houses the consciousness of the Red Skull's biochemist, Arnim Zola. In the comics, Zola transferred his consciousness to an android and has battled Cap in this form for decades. In this film, Zola was supposed to fight Cap while in his android body but the directors thought it wouldn’t match the story's tone.

At one point, the filmmakers wanted Peter Dinklage to play the antagonist, MODOK. Due to being endowed with psionic powers, MODOK is a deadly adversary who has fought Cap in the comics dozens of times. But due to his gargantuan head, the producers thought MODOK wouldn't be taken seriously and so swapped him out for a brand-new villain, Alexander Pierce.

Marvel Studios contemplated having Alexander Pierce revealed to be the Red Skull but decided it would come across as an unnecessary twist.
