One theory surrounding Heath Ledger's death from an overdose in 2008 is that getting into the mind of The Joker may have somehow led to that happening. There's no evidence to suggest that was the case, but it's a valid enough theory based on some of the claims which have been made about the way he committed to the role in such an extreme way.

Possibly fearful of something like that happening again, Warner Bros. has hired an on-set therapist/life coach to help the cast of Suicide Squad deal with the dark paths their respective roles are taking them down, and you have to believe that Leto will have paid them at least a few visits.

Adam Beach (Slipknot) revealed this information in a recent interview, adding: "[David Ayer] wants you to torment yourself. He wants the real thing," and "He wants to make sure we finish a movie and don’t disappear somewhere and then don’t show up for work."

That points to Suicide Squad being one dark and twisted movie then, and evidence suggests that Leto's Joker will be right at the heart of that.
