Avengers: Infinity War was never not going to be top of this list. Thanos completed his quest to collect the Infinity Stones and when he snapped his fingers he killed half of all sentient life in the entire universe. There is no possible way to calculate how many this killed, but on Earth alone it would have been 3.5 billion souls.

Even before this, Thanos killed more than most other MCU villains. He single handedly took the lives of Heimdall, Loki, Gamora, and Vision, as well as the majority of the Asgardians aboard the Grandmaster's ship, and half of the population of Gamora's planet, Zen-Whoberi.

It wasn't just Thanos that killed in this movie however. One of the most epic battles in MCU history took place in Wakanda while Shuri tried to extract the Mind Stone from Vision's forehead. Almost 900 Outriders and Wakandans were killed, with the former heavily outweighing the latter.

The Mad Titan also lost all four of his Black Order as Ebony Maw was shot into space, Cull Obsidian was torched by the Wakandan barrier, Proxima Midnight was thrown into one of the Threshers by Wanda, and Corvus Glaive was impaled with his own weapon by Vision. Yes, the snap put Infinity War in a league of its own with regard to deaths, but even without it, this movie would have still been one of the most deadly.
